The Code of Conduct refers to a "Report Post" feature of the web site. Where is that?

The little triangle up in the right hand corner of any post can be used to report the post. I've attached a screenshot pointing the symbol out.

Man, that is one hard to find icon. I know I've searched for exactly this multiple times.

BTW, this points to a text-only input box. Can we change this to a radio button list with N common choices and maybe one that is "other" (which has the text attached)? People are far more likely to select one of the choices than actually type something.
